The webinar reviews Domain 3 (“Culture of Safety and Learning System”) of the CMS Patient Safety Structural Measure (PSSM), a structural attestation measure to be entered in NHSN in spring 2026 and publicly reported on Care Compare in fall 2026 (required for PPS hospitals; optional for critical access hospitals). The presenter outlines objectives: define a culture of safety, explain learning systems and continuous improvement, and promote a non-punitive/just culture that encourages reporting.<br /><br />Five required elements are discussed. (1) Conduct a validated hospital-wide safety culture survey annually or every two years (with pulse surveys in off years), share results with staff and the governing board, and use findings for unit-level harm-reduction interventions. (2) Maintain a dedicated team to analyze serious safety events using an evidence-based method (e.g., RCA²), while protecting involved staff through just culture and follow-up support. (3) Use a patient safety metrics dashboard with external benchmarks to track key harms and inform improvement; share with leadership and the board. (4) Implement at least four of seven high-reliability practices (e.g., safety huddles, leadership rounding, data infrastructure, CPOE/BCMA, improvement methodology, teamwork training, human factors). (5) Participate in a patient safety learning network and implement at least one best practice.<br /><br />Resources and upcoming sessions are provided.
